The Hospital to Home program has made its way to Ingersoll and Tillsonburg.
INGERSOLL/TILLSONBURG - A new hospital program has been rolled out in Oxford County.
Both Alexandra Hospital and Tillsonburg District Memorial Hospital have been added to the Hospital to Home program, which is supported by the Ministry of Health. The program is designed to promote safe and timely transitions from hospital to home for eligible patients, providing 16 weeks of wrap-around services to be delivered in their home or community setting following discharge. Oxford County MPP, Ernie Hardeman, shares his excitement about the news.
"Our government is committed to ensuring that Ontarians receive the right care, in the right place, at the right time. This initiative will not only improve patient outcomes but also strengthen the health care system here in Oxford County by reducing pressures on our hospitals and supporting families close to home."
CEO and President of AHI and TDMH, Nadia Facca, knows this will only strengthen both hospitals and the care they give.
"Together, we are committed to providing seamless, compassionate care to support patients every step of the way from hospital to home."
The program began in over 8 hospitals, and both in Oxford County are a part of 5 additional hospitals added to the program.
