OXFORD COUNTY - Some positive news for Woodingford Lodge in Oxford County, as they are the recipients of the Future Innovator Award by The Change Foundation.
The Family Transition Program helps to ease the transition to long term care before move in day for future residents.
The purpose of the program is to support the individual and their family during the long-term care admission process by providing a unique, supportive and transitional model of care to help the individual adjust to the long-term care experience prior to their first day of admission into a residence, all at no cost.
The Change Foundation is celebrating their 25th anniversary by honouring those who have a strong vision for positive change for patients and caregivers and have the potential to make notable impact going forward.
