Tillsonburg District Memorial Hospital has opened a transitional care unit with funding from the Ministry of Health.
TILLSONBURG - A new transitional care unit is open at Tillsonburg District Memorial Hospital.
The unit has 12 beds and was made possible through 1.6-million dollars in funding from the Ministry of Health. It opened on April 1st and will operate for one-year, which will end on March 31st of next year. Patients that use the unit will have medical oversight, which supports them in the transition period from hospital to home, or to another level of care such as Long-Term Care. The unit has staff that includes, nurses, personal support workers, a recreational therapist, occupational therapist, physiotherapist assistant and physiotherapist.
Integrated CEO and President AHI and TDMH, Nadia Facca says this unit is a great collaboration.
"We are very proud to partner with Ministry of Health and our TDMH Foundation (for equipment needs) to open the transition care unit within TDMH. We know that our patients, families and communities will be well served by adding these beds to our complement. Our team is very excited to be part of this initiative."
The unit has seen great success since opening as patients have been able to transition to LTC, retirement homes or safely returning home with the support of community services.
